Filing a Grievance:

You may submit a grievance in person, by mail, fax, or email. Our Doctor and staff are available to assist you in filing a grievance.

Filing a Civil Rights Complaint:
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR), either electronically, by mail, or by phone.
